A message from the clinic:

WHAT TO EXPECT - BEFORE YOUR VISIT:

A Care Coordinator will call you in advance of your visit to ask questions regarding your health history and details regarding your current illness/injury. It's critical that you take this call or video chat before your provider visit.

WHAT TO EXPECT - AFTER YOUR VISIT:

WORK STATUS: A Care Coordinator will email you and your employer your Work Status report containing any restrictions/limitations.

FOLLOW-UP APPT: All follow-up appointments will appear on the emailed Work Status.

MEDICATION & DME ORDERS: Any medications or DMEs that have been ordered may be picked up curbside at any Reliant Location. The Care Coordinator will arrange for the order to be ready.
