Thank you for trusting us with your care at Brookshire Family Practice. We are located in downtown Brookshire near the Wells Fargo Bank with accessible parking around the building. To minimize wait time, please complete the new patient forms before your arrival(www.brookshirefamilypractice.com). Please note that you will need to present an ID, Insurance card(if applicable) and a list of all current medications upon your initial visit. We accept most commercial insurance plans, health market plans and medicare. The self pay prices for an office visit starts at $150 for new patients.
