This Consumer Health Data Privacy Notice (“CHD Privacy Notice”) describes the Consumer Health Data that Solv Health, Inc. (“we” and “us”) collects, uses, and discloses when you engage with our direct-to-consumer services, including our website at www.solvhealth.com and our consumer-facing mobile app (together, the “Service”).

We encourage you to read this CHD Privacy Notice closely. Please note that this CHD Privacy Notice does not apply to any PHI that we process in our role as a business associate to HIPAA covered entity providers.

At this time, this CHD Privacy Notice only applies to residents of Nevada and Washington. Review our Privacy Policy to understand our general practices for processing personal information.

1. Sources of Consumer Health Data

We collect Consumer Health Data in the following ways:

• Information you provide to us. Consumer Health Data and other information you may provide to us when you use our Service or otherwise interact with us.
• Information that we obtain from third party sources. We may receive Consumer Health Data about you from third party sources, such as your healthcare provider.

2. Consumer Health Data We Collect

Depending on how you use our Service, we may collect the following categories of Consumer Health Data:

• Health status or treatment information, such as your health-related conditions, symptoms, status, testing, or treatments (including surgeries, procedures, medications, or other interventions), which may include behavioral health information.
• Healthcare payment, including information about your health insurance coverage, including relevant ID numbers, copay, and billing information.
• Inferences about health derived from other health or non-health data, including the fact that you may be visiting our Service to seek healthcare.

3. How We Use Consumer Health Data

We may use the Consumer Health Data we collect for a variety of business purposes as described below:

• To manage our relationship with you, including responding to your requests or inquiries, giving you access to content or information you request, and better understand your interests and needs.
• To analyze, personalize, and improve our Service and business.
• To market and advertise our products and Service, including via email, SMS, and through direct advertisements.

We share all the categories of Consumer Health Data that we collect with the categories of recipients described below:

  • Companies that provide technical business support such as data storage, software suites, cybersecurity services, and information technology troubleshooting services; and

We may share your Consumer Health Data with third parties to comply with law or a legal process when we, in good faith, believe that disclosure is necessary or appropriate to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.
