We are only able to schedule Pfizer vaccinations for people who meet these criteria. **You must bring your vaccine card with you to your appointment.

CRITERIA FOR ALL PFIZER DOSES:

• I am age 12 or older.
• I have not received convalescent plasma or monoclonal antibodies in the past 90 days.

PFIZER BIVALENT BOOSTER CRITERIA:

• I am 12 years or older, and
• It has been at least five months since I completed my primary two-dose COVID-19 vaccination of either the Pfizer vaccine or the Moderna vaccine, or
• It has been at least two months since I received the one-dose Johnson & Johnson/Janssen (J&J) vaccine.

If you schedule a vaccination without meeting the criteria for the dose you are receiving, your appointment will be cancelled.
