BE AWARE!!!!! Do not go here. I went for a Covid test back in July. I received a bill from them for my deductible, I called my insurance and they said per their policy and the Cares Act the deductible should be waived. I called AMF Gardendale and let them know. Just received a notice they sent me to collections. I did not receive any statements other than that one I called about. I can’t get anyone on the phone. According to BBB this is a common issue with AFC. They charge for Covid testing even if it’s against the cares act and patients are getting surprise bills or collection notices. They didn’t follow any of the rules regarding sending someone to collections, never received the proper amount of statements, was never contacted etc. I tried calling, they literally will not answer the phone. I am definitely appealing this. Be weary of this location.

My husband went in for a DOT physical. He showed up at a little after 3 and was there until they were getting ready to close, still did not get his physical. After sitting in a room for over 2 hours, he walked out upset and was just going to leave. The nurse was just standing around doing nothing, she saw him leaving and stated she was just about to come in the room. He told her just like he told the person that took his vitals they would have to call his Cardiologist for them to fax paperwork needed for them to pass his physical but bc no one did their job, the cardiologist was closed so the physical could not be completed. He missed half a day of work and had to miss another to go somewhere else the next day. All the nurse kept saying was she was sorry but he could tell she really didn't care. We find it odd that the white gentleman that came in after him also for a DOT physical had come and gone while my husband ended up leaving there without what he needed. We've since been informed by others that is the worse location to go to, especially for a DOT physical. At one time they were even turning people away, telling them they didn't do DOT physicals until someone from one of the other AFC locations called and questioned them about it.
