We are located on Welborn rd next to the Prosperity Bank. Complete your online paperwork before your appointment. Please note that our last patient must be checked in by 8:30pm.

We do take most insurances but we do not offer workers comp visits. Please note that HMO plans will require a referral from your PCP. If you have Medicaid but don't have your card or subscriber number you will need to call TMHP to obtain that information at 1-800-335-8957. If you do not have active insurance or we don't take your insurance our self-pay price for an office visit starts at $129.
