Thank you for trusting us with your care. We are located in the office suite of Halo Wellness Center, at the Southeast corner of South Rainbow Boulevard and West Oquendo Road. To minimize wait time, please complete paperwork emailed to you before arrival. Please bring a government-issued ID and your insurance card. Straight Nevada Medicare, Nevada straight Medicaid, United Health AARP Medicare HMO and United Health supplemental, United Health Commercial, Aetna, and First Health are the only insurances we are able to accept at this time. All others will have to be self pay. The fee for initial consultation is $180. We welcome walk-ins. If you have any questions, our phone number is (702) 522-6108
