Visit Info - Here is a list of things you’ll need to bring to your visit: 1) a valid form of identification, 2) an insurance card, 3) a method of payment, 4) a list of known allergies and medications, and 5) a mask for face covering.

COVID-19 Testing - We are required to complete a medical evaluation prior to testing. A provider will confirm your medical history, perform a physical exam and obtain samples for lab tests, which may include a nasal swab.

My wait was unacceptably long. I checked in at 1:40 for a 2pm appt. I was not taken back until 3. Then after the dr finally saw me I waited 3” minutes before going out to see what was taking so long with my discharge paperwork. The lady behind the desk was extremely rude. The dr had forgotten to release my paperwork.
