Patient should check-in 10 minutes prior to their appointment and patients who arrive more than 15 minutes late for their appointment is subject to a cancelation of appointment and may need to be rescheduled. Must bring employee ID and/or Driver’s License. Check-in with Business Office. Parking lot located behind the facility. Review COVID-19 vaccine fact sheet . Follow after vaccine monitoring instruction – allow 15 minutes of monitoring after vaccination is administered. Wear facial covering and follow social distancing requirements.
