Rybelsus is the only FDA-approved oral semaglutide tablet. It treats type 2 diabetes in adults and contains the same active ingredient as Ozempic and Wegovy — but as a once-daily pill rather than a once-weekly injection. Oral semaglutide is convenient, but it requires strict timing on an empty stomach and produces somewhat smaller A1C and weight changes than injectable semaglutide at standard doses.1,3,4

What is oral semaglutide?

Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist. It mimics a gut hormone that boosts insulin release after meals, slows stomach emptying, and signals fullness to the brain. Rybelsus is the only FDA-approved oral formulation; it pairs semaglutide with a permeation enhancer (SNAC) that helps the drug cross the stomach lining.1,2

Ozempic and Wegovy are the same molecule in injectable form. Ozempic is approved for type 2 diabetes and cardiovascular risk reduction; Wegovy is approved for chronic weight management.3

Who is Rybelsus for?

Rybelsus is FDA-approved for adults with type 2 diabetes, used along with diet and exercise to improve blood sugar control. It is not approved for:1

Like other GLP-1s, Rybelsus carries a boxed warning about thyroid C-cell tumors observed in rodents. It should not be used by people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2.1

How do you take Rybelsus?

Dosing follows a step-up schedule, with at least 30 days between each step:1

  • Days 1–30: 3 mg once daily (this dose is for titration only; it does not lower A1C meaningfully)
  • Day 31 onward: 7 mg once daily
  • After another 30 days if needed: 14 mg once daily

Strict administration rules — these matter for absorption:1,5

  • Take the tablet on an empty stomach, when you first wake up
  • Use no more than 4 ounces of plain water — no other liquid
  • Wait at least 30 minutes before eating, drinking anything else, or taking other oral medications
  • Swallow the tablet whole — do not crush, chew, or split

How does oral semaglutide compare to injectable?

The injectable form is somewhat more effective at standard doses:4

  • A1C reduction: Injectable semaglutide 1.0 mg lowers A1C by roughly 1.5 to 1.8 percentage points. Oral semaglutide 14 mg lowers A1C by roughly 1.0 to 1.4 percentage points.
  • Weight loss: Injectable semaglutide produces greater weight reduction than the 14 mg oral dose in clinical trials.
  • Dosing: Oral is daily; injectable is once weekly.
  • Cost: Insurance coverage varies; injectable is more widely used in real-world practice.

For some patients — especially those uncomfortable with injections or who prefer daily dosing — oral semaglutide is the better fit. Others benefit from the larger A1C and weight changes of the injectable.

What are the side effects of oral semaglutide?

Common

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain
  • Decreased appetite

GI symptoms are most common during dose escalation and usually improve over four to eight weeks.5

Serious (call your clinician right away)

  • Severe abdominal pain, especially if it radiates to the back (possible pancreatitis)
  • Yellowing of the skin or eyes (possible gallbladder problem)
  • Severe persistent vomiting or signs of dehydration
  • Fast or pounding heartbeat
  • A neck lump or hoarseness (possible thyroid issue)
  • Symptoms of low blood sugar if you also take insulin or a sulfonylurea

Who shouldn't take oral semaglutide?

Avoid Rybelsus if you have:1

  • Personal or family history of medullary thyroid carcinoma
  • Multiple endocrine neoplasia syndrome type 2
  • A known allergy to semaglutide or any of its ingredients
  • A history of pancreatitis (use with caution)

Tell your clinician if you're pregnant, planning to become pregnant, or breastfeeding. Semaglutide should generally be stopped at least two months before a planned pregnancy.

Is Rybelsus approved for weight loss?

No. Rybelsus is approved only for adults with type 2 diabetes. Off-label prescribing for weight loss occurs, but insurance rarely covers it for that indication. Wegovy (injectable semaglutide) is the FDA-approved option for weight loss.1,3

Why do I have to take Rybelsus on an empty stomach?

What if I miss a dose of Rybelsus?

Skip the missed dose and take the next dose at your usual time the next day. Don't double up. If you miss two or more consecutive doses, talk to your prescriber — restarting may require dose adjustment.5

Are oral and injectable semaglutide interchangeable?

Not without prescriber input. The conversions are not 1:1 by milligram, and switching requires re-titration. Your clinician will choose based on your tolerance, preference, blood sugar response, and insurance coverage.
