Scheduled the appointment online and it showed me the address of 3555 Northlake Blvd, and apparently you have moved. Drove in circles for awhile until I finally called the number and she directed me to your current location. Got there 15 minutes late due to the incorrect  information. The receptionist was helpful but was sick and coughing and sniffling. I did not come here to get sick! Other than that I was seen quickly, the PA was very nice and the rest was a good experience. Maybe update your new location with Medicare as well as your website?
